What is an EJ line?
A peripheral IV catheter inserted into the external jugular vein is considered a peripheral IV, often referred to as EJ PIV. The EJ PIVs are used for emergent access or for individual situations when other veins cannot be accessed.
How do you place an EJ?
Procedure
- Place patient in Trendelenburg position and rotate head to opposite side of cannulation.
- Position yourself at the head of the bed facing the patient.
- Clean skin with appropriate antiseptic.
- Use non-dominant thumb to provide counter-traction and index finger to tamponade EJV just superior to clavicle.
What is the CPT code for IV placement?
According to the CPT hierarchy, the initial code must be 96374. Following that code, 96361 must be assigned for the hydration. Sole purpose of the IV fluid administration is to keep vein open or maintain vascular access, this should not be separately reported.

It is the position of the Infusion Nurses Society that a qualified licensed registered nurse may insert, care for, maintain, and remove EJ PICCs and EJ PIVs. Once the registered nurse has validated initial competency, there must be a program in place to support ongoing clinical competency.
How do you bill for IV infusion?
Assign CPT 96360- IV hydration, initial 31-90 minutes, and CPT 96361 (add on code), used once infusion lasts 91 minutes in length. An intravenous infusion of hydration of 30 minutes or less is not billable. Hydration infusion must be at least 31 minutes in length to bill the service.
